All you need to know about the Chughtai Lab Gift Card:

• There are 3 categories.
  

  Gift Card A Rs 10,000

  Gift Card B Rs 20,000

  Gift Card C Rs 30,000

• Who can use the prepaid gift card?
  The prepaid gift card can be used by the registered recipient at the time of purchase.
  The registered recipient can add up to 10 members. The total user numbers cannot exceed 9+1 members.
• Can I recharge the prepaid gift card online?
  In order to recharge the card, you will have to pay the chargeable amount at any of our centers.
• This Card will be activated within 48 hours of purchase.

Terms and Conditions:

• Chughtai Lab Gift Card is non-transferable. Only Chughtai Lab Gift Card members listed on the card account may use the Gift Card.
• Chughtai Lab Gift Card benefits at health care providers other than Chughtai Lab are at the sole discretion of that provider.
• Chughtai Lab shall bear no liability if a third-party healthcare provider opts out of honoring the Chughtai Lab Gift Card.
• If you lose your Gift Card, please notify us immediately so that a replacement card may be issued. Replacement card fees will apply.
• Chughtai Lab reserves the right to alter/discontinue the Gift Card program or individual Gift Cards at any time at its sole discretion.
• Special sales and promotions may not be eligible for discounts. Only one offer can be availed of at a time.
• Chughtai Lab may decide not to offer/discontinue discounts on certain tests/procedures.

Validity: 3 years
