Blue Card

Chughtai Lab is pleased to announce the Blue Card program to facilitate patients. Card holders can avail discounts at Chughtai Lab and other participating health care establishments. In addition, card holders can earn points that may be redeemed at Chughtai Lab at a later time.

How to get your Blue Card?

You can purchase your blue card for against a payment of Rs 500. Your personalized Blue Card shall be mailed to you at your address within fourteen (14) business days.

8% Discount on Medicines
15% Discount + 5% points on Lab test investigations
15% Discount on Radiology

Validity

The Blue card will be valid for a period of 1 year from the date of issuance. You may renew it at any time against a payment of Rs. 200. In case of loss, please call us to get a replacement Blue Card*.

Types of Blue Cards:

• Individual (Single) Blue Card      Rs. 500/-
• Family Blue Card                        Rs. 1000/-
  (6+1 maximum of 7 members including the Blue card holder)

Terms and Conditions:

BLUE CARD is non-transferable. Only BLUE CARD members listed on the card account may use the BLUE CARD.
BLUE CARD members earn Rewards Points for payment made at Chughtai Lab. Each Reward Point equals PK Rs. 1/-. Points can be redeemed after your points balance exceeds more than 500 points anytime.
The minimum points balance should be 100 points.
The BLUE CARD is valid for one year from the date of issuance. Blue Card may be renewed against renewal fee payment upon member request.
BLUE CARD benefits at health care providers other than Chughtai Lab are at the sole discretion of that provider.
Chughtai Lab shall have bear no liability in the event of a third party healthcare provider opting out of honoring BLUE CARD.
If you lose your BLUE CARD, please notify us immediately so that a replacement card may be issued. Replacement card fees shall apply.
Chughtai Lab reserves the right to alter / discontinue the BLUE CARD program or individual Blue Cards at any time at its sole discretion.
Special sales and promotions may not be eligible for discount / points. Only one offer can be availed at a time.
Chughtai Lab may decide to not offer/discontinue discounts on certain tests/procedures.
