All you need to know about the Chughtai Lab Gold Membership Card:

• There are 2 categories.
  

  Membership Card A Rs 10,000

  Membership Card B Rs 20,000

• The prepaid gold membership card can be used by the registered recipient at the time of purchase. The registered recipient can add up to 10 members. The total user numbers cannot exceed 9+1 members.
• In order to recharge the card, you will have to pay the chargeable amount at any of our centers.
• This Card will be activated within 48 hours of purchase.
• Top-up options on Chughtai Lab Gold Membership Card are Rs. 5,000, Rs. 10,000, and Rs. 20,000.

Terms and Conditions:

• Chughtai Lab Gold Membership Card is non-transferable. Only Chughtai Lab Gold Membership Card members listed on the card account may use the Gold Membership Card.
• Any amount added on Chughtai Lab Gold Membership Card is non-refundable. 
• Chughtai Lab shall bear no liability if a third-party healthcare provider opts out of honoring the Chughtai Lab Gold Membership Card.
• If you lose your Gold Membership Card, please notify us immediately so that a replacement card may be issued. Replacement card fee of PKR 1000 will apply.
• Special sales and promotions may not be eligible for discounts. Only one offer can be availed of at a time.
• Chughtai Lab may decide not to offer/discontinue discounts on certain tests/procedures.
• Chughtai Lab reserves the right to alter/discontinue the Gold Membership Card program at any time and or any applicable terms, at its sole discretion. 
• Incase of any material change Chughtai Lab will notify the same on registered mobile and will consider your acknowledgement if no reservation received from user side within 24 hrs.

Validity: Lifetime
