How to get your Gold Membership Card?

To apply for the Gold Membership Card, the patient has to visit the nearest branch of Chughtai lab and fill out a
Gold Membership Card request form. The card will be handed over on the spot to the patient or call Aap Ki Sehat Ka

Is the Gold Membership Card facility available all over Pakistan?

Yes, we are offering Gold Membership Card facility in all Chughtai lab branches of Pakistan.

Is Gold Membership Card transferable?

Chughtai Lab Gold Membership Card is non-transferable. Only Chughtai Lab Gold Membership Card members listed on the
card account may use the Gold Membership Card.

What should I do if I lost the Gold Membership Card?

If you lose your Gold Membership Card, please notify us immediately so that a replacement card may be issued.
Replacement card fees will apply. 

Can I use the discounts on top of the Gold Membership Card?

Special sales and promotions may not be eligible for discounts. Only one offer can be availed of at a time.

Who can use the prepaid Gold Membership Card?

The prepaid Gold Membership Card can be used by the registered recipient at the time of purchase.

The registered recipient can add up to 10 members. The total user numbers cannot exceed 9+1 members.

Can I recharge the prepaid Gold Membership Card online?

In order to recharge the card, you will have to pay the chargeable amount at any of our centers.

This Card will be activated within 48 hours of purchase.

• Chughtai Lab Gold Membership Card is non-transferable. Only Chughtai Lab Gold Membership Card members listed on the card account may use the Gold Membership Card.
• Any amount added on Chughtai Lab Gold Membership Card is non-refundable.
• Chughtai Lab shall bear no liability if a third-party healthcare provider opts out of honoring the Chughtai Lab Gold Membership Card.
• If you lose your Gold Membership Card, please notify us immediately so that a replacement card may be issued. Replacement card fees will apply.
• Special sales and promotions may not be eligible for discounts. Only one offer can be availed of at a time.
• Chughtai Lab may decide not to offer/discontinue discounts on certain tests/procedures.
• Chughtai Lab reserves the right to alter/discontinue the Gold Membership Card program at any time and/or at any applicable terms, at its sole discretion.
• In case of any material change, Chughtai Lab will notify the same on registered mobile number and will consider your acknowledgment if no reservation received from the user’s side within 24 hours.
