What is a calcium blood test?
Calcium is an essential mineral in the human body. Your blood contains around 1% of the calcium in your body, while teeth and bones store 99% of it. The right amount of calcium in your blood is essential for the proper functioning of your heart, muscles, and nerves. Additionally, it facilitates the release of hormones that impact several bodily processes and helps blood vessels transport blood throughout the body.
A calcium blood test - also known as a serum calcium test or S. calcium test - quantifies the amount of calcium circulating in your blood. It is routinely included in basic and comprehensive metabolic panels (BMP/CMP) ordered during regular checkups, hospital admissions, and pre-operative evaluations.

Types of calcium tests - which one do you need?

As part of your regular examination, your doctor may have prescribed a basic or comprehensive metabolic panel, which includes a calcium blood test. Additionally, if you have symptoms of abnormal calcium levels or disorders that can affect your blood calcium, you may undergo this test to monitor or identify them.
Symptoms of high blood calcium (hypercalcemia)
- Digestive symptoms such as constipation, abdominal pain, loss of appetite
- Increased thirst and urinating more than usual
- Kidney stones
- Fatigue and general weakness
- Bone, joint, and muscle pain
- Confusion, depression, or changes in mental health
Symptoms of low blood calcium (hypocalcemia)
- Dry skin, coarse hair, brittle nails
- Muscle cramps, spasms, or stiffness
- Irregular heartbeat
- Tingling in the lips, tongue, fingers, and feet
- Seizures in severe cases
Many individuals with either high or low serum calcium levels show no symptoms at all. This is why calcium is included in routine metabolic panels - it can detect problems before symptoms develop.
Your doctor may also prescribe a calcium test if you have a known condition that affects calcium levels, such as kidney disease, thyroid or parathyroid disease, malnutrition, Vitamin D deficiency, or certain cancers.
How to prepare for a calcium blood test
If a serum calcium test is your only test, no fasting is required. You can eat and drink normally before the test.
However, the calcium test is often included as part of a basic metabolic panel (BMP) or comprehensive metabolic panel (CMP), which measures multiple substances in your blood at once. In this case, your doctor may ask you to fast for 8 to 10 hours before the test.
Additional preparation notes:
- Inform your doctor or the collection staff about all medications you are taking, including calcium supplements, antacids, diuretics, and Vitamin D supplements - these can affect your results
- Avoid calcium supplements for 24 hours before the test if instructed by your doctor
- Wear a short-sleeved shirt to allow easy access for blood collection
- Drink plenty of water before the test - being hydrated makes blood collection easier
The risks of the test are minimal. You may experience slight discomfort or bruising at the needle insertion point, which typically resolves quickly.
Normal calcium levels - what does your result mean?
Calcium test results are measured in milligrams per deciliter (mg/dL). Reference ranges can vary slightly between labs:

What does a high calcium level mean?
If your test results show that your blood calcium levels are higher than normal (hypercalcemia), your doctor may investigate for:
- Hyperparathyroidism (overactive parathyroid gland) - the most common cause in outpatients
- Hyperthyroidism
- Kidney failure
- Tuberculosis or sarcoidosis
- Addison's disease
- Lung cancer, breast cancer, or certain blood cancers
- Metastatic bone tumors or Paget's disease of the bone
- Multiple myeloma
- Vitamin D intoxication from excess supplementation
- Prolonged immobilization
If your results are high, your doctor may request follow-up tests including parathyroid hormone (PTH), Vitamin D, phosphorus, and urine calcium before making a diagnosis.
What does a low calcium level mean?
If the results show that your blood calcium levels are below normal (hypocalcemia), your doctor may investigate for:
- Hypoparathyroidism (underactive parathyroid gland)
- Vitamin D deficiency - extremely common in Pakistan
- Hypomagnesemia (low magnesium)
- Osteomalacia (softening of bones)
- Pancreatitis
- Kidney failure or chronic kidney disease
- Liver disease causing low albumin (in this case, corrected calcium test is more accurate)
- Celiac disease or other malabsorption conditions
Note: A calcium blood test does not tell you how much calcium is in your bones. To assess bone mineral density, a DEXA scan is needed. Low blood calcium and low bone density are related but different findings - you may have low bone calcium (osteoporosis) with normal blood calcium levels.

Pakistan has one of the highest rates of Vitamin D deficiency in the world. This matters directly for your calcium test results because Vitamin D is required for calcium absorption from the intestine. Without adequate Vitamin D, your body cannot absorb calcium from food properly - even if your diet is calcium-rich.
This means:
- Low blood calcium in Pakistani patients is frequently caused by Vitamin D deficiency, not just low calcium intake
- If your calcium is low, your doctor will almost always also check your Vitamin D level
- Treating low calcium without also correcting Vitamin D deficiency is often ineffective
- Calcium supplements alone will not raise blood calcium if Vitamin D is severely deficient
Women who are pregnant, breastfeeding, or covering their skin outdoors are at particularly high risk of both Vitamin D and calcium deficiency in Pakistan. If you are in these groups, discuss screening with your doctor even without obvious symptoms.
