Purpose of the Test

The test is ordered when a patient has symptoms of a lung infection such as coughing, shortness of breath, fever, and chest pain. It is also used to diagnose pneumonia, bronchitis, and other respiratory infections.

When this test is required

The test may be requested when a patient has symptoms of a lung infection such as coughing, shortness of breath, fever, and chest pain, and other tests such as chest x-ray or blood cultures have not provided a definitive diagnosis. It may also be used to monitor the effectiveness of antibiotic therapy.

What the Test Detects

Bronchoalveolar lavage (BAL) for Gram stain is used to identify bacteria in the lungs. The test involves staining a sample of fluid obtained from the lungs with a special dye that makes bacteria visible under a microscope.

Preparation for the Test

Before the procedure, the patient may be given a sedative and/or local anesthesia to help them relax and minimize discomfort. It is important to follow any specific instructions provided by the healthcare provider prior to the procedure, such as fasting or temporarily stopping certain medications.

Sample Requirements

To perform a BAL, a small amount of saline solution is introduced into the lung through a bronchoscope, a thin tube with a light and camera on the end. The saline is then suctioned back out and collected in a sterile container for laboratory analysis.

BAL is a relatively safe procedure, but it does carry some risks, including bleeding, infection, and lung collapse. It is important to discuss the risks and benefits of the procedure with your healthcare provider. Additionally, the test may not always provide a definitive diagnosis, and additional tests may be necessary.
